About the Caring Hearts Program
ThedaCare is committed to improving the health of our communities. Caring Hearts, our financial assistance program, is designed to help patients who are unable to pay for medically necessary medical services. The Caring Hearts program will reduce or eliminate the patient bill if eligibility criteria are met. No one will be denied access to services due to inability to pay and there is a discounted/sliding fee schedule available based on family size and income.
Application Process
Complete the Caring Hearts application and provide us with the requested financial documentation so we can determine your eligibility. Please understand we cannot process your application without the financial documentation.
Obtaining an Application
Call us at (800) 236-4102 to request an application so we can update your account. This will avoid collection action if your account is past due.
Results
Our Customer Support experts will process your application according to the guidelines outlined in our policy (see below). Provided you supply all of the requested information, you will receive the results in writing within 10 business days. If you receive a partial discount on services, please contact us and we will work with you to set up a payment plan for the remaining balance.

Elevate Patient Financial Solutions
ThedaCare partners with ElevatePFS to provide free financial advocacy for uninsured patients who
have incurred medical expenses within their hospitals and clinics. ElevatePFS’s patient-focused approach
identifies the most appropriate assistance from a variety of resources that can help you pay for your medical
bills. You may have been referred to ElevatePFS for outreach if you have recent visits at ThedaCare
with no insurance listed on file, or were identified with additional coverage needs by a financial counselor.
