Safe, Secure Access on MyThedaCare
A MyThedaCare account helps you schedule in-person appointments or log on for virtual visits. This secure online health care portal allows you to communicate with your care team, access test results, request prescription refills, manage your appointments and more. What is Included in a Wellness Visit?
A wellness visit is all about keeping you well. It is a routine preventive health check-up that focuses on your current health, risk factors, family history, and preventive care. The goal is to help prevent or detect health problems before they develop into serious disease.
Your visit may include:
• Screening tests, defined as tests to detect early disease or risk factors for disease, such as diabetes, cancer, and high cholesterol
• A review of your health history
• A physical exam
• ImmunizationsWhen Might Additional Charges Apply to a Wellness Visit?
Add-ons to a wellness visit are services related to pre-existing or new medical problems, and they can result in additional charges for your visit. Wellness visits do not include treatment of acute concerns (such as new symptoms) or chronic medical conditions.
Add-ons to a wellness visit may include:
• Evaluation of known chronic medical conditions (like diabetes, asthma, depression, or high blood pressure)
• Orders for non-screening imaging or lab work
• Interpretation of a non-screening test
• When a new medication is ordered or an existing prescription is refilled
• When a new diagnosis is made
• When a procedure is performedCare Plus Convenience
A wellness visit can be combined with an acute care visit or a chronic disease visit for your convenience. However, additional charges may apply in accordance with your health care coverage. If you have a combined visit (wellness + treatment), we will submit a bill to your insurance which includes both charges. Please remember:
• You can get help in a timely manner for acute or chronic conditions throughout the year. You don’t have to wait for your annual wellness visit and try to do it all at once.
• Tell our scheduler when you call for your wellness visit if you will need extra time to discuss other concerns. This also helps you plan what questions you want to bring up at your appointment. If you prefer, you can schedule two separate visits.
• You and your insurance company may be charged for services and co-pays if you receive additional health care services at the same time as your wellness appointment.Medicare Part B Medical Insurance covers a “Welcome to Medicare” preventive visit once within the first 12 months you have Part B. The visit includes a review of your medical and social history related to your health and education and counseling about preventive services, including:
- Certain screenings, flu and pneumococcal shots and referrals for other care, if needed
- Height, weight and blood pressure measurements
- A calculation of your body mass index
- A simple vision test
- A review of your potential risk for depression and your level of safety
- An offer to talk with you about creating advance directives
- A written plan letting you know which screenings, shots and other preventive services you need
Please organize important paperwork and insurance information prior to your first appointment. Be sure to:
- Plan to accompany your minor child to his or her first appointment to complete registration and consent forms. If he or she will come to appointments alone in the future, please sign a Consent for Treatment form when you come to the first visit.
- Contact your insurance company to confirm your coverage. Questions to ask may include the frequency of covered wellness exams and what services are covered in a wellness exam. When you confirm coverage in advance, you will better understand the potential for additional out-of-pocket expenses for which you will be responsible.
- Plan to make insurance co-payments (co-pays) at the time of your visit.
- Bring your insurance card to every appointment.
All traditional clinic appointments must be made in advance. If you must cancel or reschedule to an appointment, the clinic must be notified at least one hour before your scheduled appointment time.
As of June 1, 2022 all prescription refill requests should be submitted via your MyThedaCare online healthcare portal.

Good on the Go. MyThedaCare can be accessed from your smart devices using the MyChart app.Did You Know? Use your name, not your prescription number. When you request a prescription refill, many pharmacies automatically assign a new prescription number. Please ask pharmacy staff to check if your prescription is ready under your name, not your (old) prescription number. You’ll save time and avoid having to call your provider to check on your refill request.

If your lab(s) require special preparation, you will be prompted to schedule by phone.
Preventative Care Helps
At ThedaCare Physicians, your primary care provider will help you manage your health and prevent disease. Routine checkups and screenings are vital as part of your preventative care plan.
Recommended Screenings for Both Men and Women:
- Cholesterol: Every one to five years, based on doctor recommendations
- Blood pressure: Every 3–5 years if you are age 18–39, once a year if you are age 40 or over
- Vision and hearing: Once a year
- Dental checkups: Once or twice a year
- Colonoscopy: Begin at age 50 and screen every 10 years; African-Americans begin at 45
- Diabetes: People age 40–70 should be screened regularly
- Welcome to Medicare: Medicare Part B (Medical Insurance) covers a “Welcome to Medicare” preventive visit once within the first 12 months you have Part B.
Important Screenings for Women:
- Cervical cancer: Have a pelvic exam and Pap test starting at age 21. Women age 21–30 should be screened at least once every three years; those age 30–65 should be screened every 3–5 years. Talk to your doctor about future screening, testing intervals, and testing for HPV, the human papillomavirus.
- Breast cancer: Perform monthly self-exams. Consider mammograms if you are aged 40–49. Regular mammograms are recommended for women age 50–74. If you have a family history of breast cancer, consult your healthcare provider for recommended screening age and frequency.
- Osteoporosis: Get a bone density test (a Dexa scan) to test the strength of your bones at age 65. Start earlier, if you have one or more risk factors in addition to menopause.
Important Screenings for Men:
- Prostate cancer: Check with your doctor about pros and cons of regular screening. Potential tests include a digital rectal exam (DRE) and a prostate-specific antigen (PSA) blood test.
To find out if you need other immunizations or screenings, consult your provider.
Adult Vaccinations
Adults need to stay up-to-date on their vaccines and make sure they are immune to serious diseases. Even if you were vaccinated as a child, the protections can wear off.
Consult your ThedaCare provider to find out what immunizations are best for you, at your age.
Typically, recommended vaccinations for adults are:
- Flu shot: Once a year
- Tetanus/diphtheria booster: Every 10 years
- Pneumococcal vaccine for pneumonia: Starting at age 65 or earlier if you have certain chronic diseases
- Shingles vaccine: Starting at age 50
- Travel vaccines: For adults leaving the country; required vaccines vary
- Certain adults with risk factors or immune deficiencies may also need immunizations for cervical cancer, bacterial meningitis, hepatitis, chickenpox and other illnesses.
If you have chronic illnesses, such as diabetes, heart disease and lung disease, it’s especially important to stay up-to-date on your vaccinations. People who have chronic illness are more likely to develop serious complications from the flu, pneumonia and other diseases.
Childhood Vaccinations
One of the best ways parents can protect their children from harmful diseases, pain and disability is through vaccination.
Before babies even leave ThedaCare Family Birth Care, they receive their first dose against Hepatitis B, a virus that can cause chronic swelling of the liver.
As a newborn and throughout childhood, your child will receive vaccinations on a regular schedule. The vaccines will prevent diseases, including:
- Chickenpox
- Diphtheria
- Tetanus
- Pertussis (whooping cough)
- Hepatitis A and B
- HPV: human papillomavirus (which is linked to cervical and other cancers)
- Influenza
- Measles
- Meningococcal disease
- Mumps
- Polio
- Rotavirus
- Rubella
