The Heritage—Independent and Assisted Living
Enjoy the comforts of home without the responsibilities. The Heritage Independent and Assisted Living communities offer private, personalized apartments and elegant common areas for joining in social activities like woodworking, exercise classes, crafts, games, book clubs and discussion groups. Our housekeeping, dietary, maintenance and nursing professionals are here to assist you with your needs, while encouraging your independence and personal interests. Services include cooking, cleaning, home maintenance, and personal care assistance. You’ll appreciate the security of 24-hour staffing and individualized care, while enjoying the warm, home-like environment, interesting social activities, and friendly neighbors.
Peabody Manor—Skilled Nursing Care
Get the care you need in a cheerful, nurturing environment. ThedaCare’s Peabody Manor is a Medicare-certified facility offering long-term or short-term skilled nursing care as well as rehabilitation services. The individual rooms are grouped in neighborhood-like settings that share dining and lounge areas, providing you with both privacy and ample opportunities for socialization and activity. Meals are prepared in our shared kitchen meeting each individual’s personalized dietary needs. While our priority is providing outstanding skilled nursing care, your daily living, social, and activity needs are important to us, too. Whatever you need, we’ll work with you and your family to develop a care plan that meets your immediate and long-term goals.
Juliette Manor—Skilled Nursing Care
Juliette Manor is located right next to ThedaCare Regional Medical Center-Berlin and offers a home-like setting with 37 spacious, private rooms offering short term rehabilitation care, stable long-term care and memory care. Each room features wheelchair-accessible showers and plenty of storage space. The one-story facility organizes the three population types within separate wings with shared common spaces. Gabled roofs defining each wing, clapboard siding, local vernacular material, natural lighting, gardens, and exterior courtyards contribute to the homelike setting, and amenities include a coffee shop and cafe, beauty salon, chapel, convenience store, therapy gym and more.
Senior Care Services
ThedaCare Senior Care Services provide care management to patients during their stay at select assisted living or skilled nursing settings throughout the ThedaCare Service area. This includes working collaboratively with other providers to oversee the plan of care and offering to become the primary care physician for long-term stay patients.
Advance Care Planning
As part of the Fox Valley Advance Care Planning Partnership, ThedaCare partners with other health systems in surrounding communities to support individuals and families through end-of-life decisions, including the advance care planning process. Classes are also available to help guide individuals through these challenging conversations and the development of a Power of Attorney for health care.
