Accompanying you through the treatment of cancer is only part of the journey we’ll take together. Cancer survivorship can be a complex time of transition, change and adjustment, and you might be wondering what comes next. As you adapt to the life and routines you had prior to your diagnosis, we are here to care for you, providing physical and emotional support every step of the way. In addition to scheduled follow-up appointments, we can assist with:

- Screening for and treating potential reoccurrence of the disease

Cancer Survivorship Programs
Survivorship Program
Our survivorship program team helps to determine the services and programs offered through ThedaCare and throughout the community that will help to address the unique needs of each cancer survivor. The survivorship care plan is communicated to the survivor and their family members after treatment is completed and provides a detailed record of cancer treatment history to help them coordinate future care. The plan is frequently revisited and adjusted as necessary to address changing needs of the survivor, and typically includes:
- Type, location, stage and treatments of your cancer
- Side effects or symptoms to watch for
- Contact information for your care team
- Resources for making healthy lifestyle changes
- Sources of support
LiveSTRONG Program
As a cancer survivor, you know the tremendous toll the disease and its treatment can take on your spirit, mind and body. In partnership with the Fox Cities YMCA, the free 12-week LiveSTRONG program helps cancer survivors continue to heal and reclaim their health by way of building muscle mass and strength, increasing flexibility and endurance, and improving confidence and self-esteem. Instructors guide participants through physical activity sessions to help them achieve personal wellness goals.
Cancer Care Support Groups
HOPE
The Hope Support Group provides education, empowerment and inspiration for patients and families living through and beyond cancer.
When: 3rd Wednesday of the month, 5:00 PM – 6:00 PM.

You are Not Alone
A support group designed for newly diagnosed patients, survivors, and family members of those battling breast cancer.
When: 3rd Monday of the month, 3:00 PM – 4:00 PM.

Gynecology Oncology Cancer Support Group
A support group for women in any stage of their GYN ONC cancer.
When: 3rd Thursday of the month, 4:30 PM – 6:00 PM.

SPOHNC (Support for People with Oral, Head and Neck Cancer)
Support for People with Oral and Head and Neck Cancer, Inc. (SPOHNC), is a patient-directed self-help organization dedicated to meeting the emotional, physical and humanistic needs of oral and head and neck cancer patients and their caregivers. SPOHNC offers information, support and encouragement to newly diagnosed patients, survivors, family members and friends in a friendly and non-threatening forum.
When: 4th Tuesday of the month, 2:30 PM – 4:00 PM.

Strong in Survivorship
Sessions include a nutrition education by an RDN, food sampling, guest speakers on various survivorship topics and a physical therapist leading a short session on gentle yoga/meditation.
Sign up is required, but the program is free of charge for the ThedaCare cancer survivor and one guest.
When: Group meets quarterly in March, June, September and December on the 2nd Tuesday of the indicated months from 5-6:30pm
