ThedaCare now provides an Online Patient Record Request Tool for:
- Patients requesting their own records
- Parents or legal guardians requesting records for a minor
- Caregivers or authorized representatives (e.g., Power of Attorney)
This secure tool verifies your identity using a photo of your driver’s license, which you can take using your smartphone or webcam.
There is no additional charge to use this service.

WIU Clinic Records
As of August 16, 2025, all medical record requests related to WIU clinic services, past or future, must now be submitted through our online records system.
