Working with your doctor, we will look after your physical, social and emotional needs, placing you at the center of your personalized care plan. In addition, our medical social workers provide support and resources to help you navigate finances, insurance and family issues as well.
When a physician writes an order for homecare services, one of ThedaCare’s registered nurses will visit the patient’s home to assess their needs. The nurse will ask the patient questions about his/her hospital discharge instructions, diagnosis, symptoms, medications and more.
Skilled Home Health nursing services offered:
Along with the patient’s doctor, our nurses develop a medication schedule, evaluate the effectiveness of medications and educate the patient and/or his or her caregiver on proper dosage, frequency and side effects.
For patients with chronic conditions, such as heart failure, diabetes and chronic obstructive pulmonary disease (COPD), our Disease Management programs aim to help patients maintain independence and a healthier lifestyle. Services include nursing care, blood pressure measurements, pulse, respirations, lung sounds, blood glucose or pulse oximetry, as ordered by your doctor.
After suffering from a severe injury, like burns, stroke, or amputation, patients often need rehabilitative nursing care to safely transition from a hospital or treatment center to home. The nurse will assess the patient’s abilities, then create a plan of care to address a patient’s individual needs.
To assist patients in resuming their daily routine at home, we provide support to help with daily living activities including bathing, grooming, dressing, walking, and much more.
For patients with complex, non-healing wounds or new stage III and IV pressure ulcers, our skilled nurses provide specialized, in-home care and education to ensure wounds heal.
With the guidance of our skilled nurses, patients learn how to care for an ostomy and live comfortably with an ostomy pouch.
Understanding the level of discomfort associated with incontinence, we offer comprehensive in-home continence care for patients. Services include urinary catheter maintenance, toileting programs and home accessibility assessments.
With falls ranking as a leading cause of death among aging adults, fall prevention is an essential component of the care our team provides. For individuals who have suffered a fall, or are at risk for falling, our homecare team provides recommendations and education to increase a patient’s safety at home.

Therapy Services
Working with your physical, occupational and speech therapists, you will create a plan of care to meet your personal health needs.
Home Health Care Therapy services offered:
Physical therapists work with you in our home after you’ve fallen, had surgery, or have an injury. Services include a home safety evaluation, exercise programs, recommendations for medical equipment, and home modification recommendations. Our physical therapy services can also be beneficial if you have an ongoing medical condition that has gotten worse, such as chronic heart failure or breathing problems, Parkinson’s disease or diabetes. In addition, physical therapists can help if you are dealing with weakness or poor balance after a stroke or other medical event. Physical therapists help patients’ improve their strength and stability, increase range of motion, and prevent falls or further injury.
Our occupational therapists work with you when you are recovering from upper body or hand surgery, have an ongoing medical condition that has gotten worse, such as arthritis, Parkinson’s disease or diabetes, have returned home with weakness after a stroke or other medical event or are experiencing temporary or long term pain. Occupational therapists help with activities of daily living, fine motor skills and adaptive equipment training. They also offer strategies for energy conservation and work simplification. In-home occupational therapists help patients who have physical, developmental, social or emotional limitations that prevent them from performing the general activities of daily living. Therapists instruct patients to use special techniques and equipment to improve function in basic household tasks, such as eating, bathing, and dressing.
ThedaCare’s certified speech-language pathologists help patients overcome conditions that affect their ability to communicate, swallow, and talk. They also help patients find ways to compensate for impaired understanding and expression of language. Speech therapists are essential in the recovery process for patients who’ve experienced stroke, brain trauma, injury to vocal cords, or to assist those who have problems swallowing due to ongoing medical conditions, such as Parkinson’s disease, or due to dementia. Our speech and language pathologists work with you in your home to help improve your communication skills and make dietary recommendations.
Home Health Patient Telemonitoring
ThedaCare’s Home Health Patient Telemonitoring program provides remote patient monitoring services to our home health population. This service is included as part of their home health episode of care at no cost to the patient. Patients received east-to-use monitor and biometric screening devices such a scale, blood pressure monitor and pulse oximeter. These devices are paired with an easy-to-use tablet, customized for each patient with medication management tools, educational videos, teach-back quizzes, video visits, daily symptom surveys and biometric monitoring. The use of this technology promotes continuity of care, improves clinical outcomes, and reduces hospital readmissions.
Patient monitors are customized with assigned metrics and provide timed reminders to patients to complete their testing at least once per day. Assigned metrics may include:
- Weight
- Blood Pressure
- Pulse Oximetry
- Temperature
- Glucose Wound Imaging
- Symptom and Risk Surveys
- Activity and Medication Reminders
- Diagnosis Specific Educational Videos, Quizzes and Documents
The patient’s recorded information is reviewed each day by a registered nurse and they will connect with the patient if readings indicate a change in condition or other concern. The nurse will also collaborate with the patient’s care team and provide updates to the patients physicians if needed. Patients also have access to a nurse 24 hours a day, 7 days a week via our triage line.

Home Infusion Services
Home infusion services are a cost-effective alternative to lengthy hospitalizations, admissions to a skilled nursing facility or frequent trips to an outpatient setting to receive needed IV medication. Our home infusion therapy program provides a wide range of infusion services including, but not limited to:
- Total Parenteral Nutrition (TPN)
- Antibiotics, antiviral, and anti-fungal therapy
- Continuous pain management
- Hydration
- Central catheter management
- Inotropic therapy
- Steroid therapy
One of our registered nurses will instruct you and your caregivers on how to administer intravenous fluids, antibiotics, pain medications, and nutritional supplements in the comfort of your home.
