When a physician writes an order for homecare services, one of ThedaCare’s registered nurses will visit the patient’s home to assess their needs. The nurse will ask the patient questions about his/her hospital discharge instructions, diagnosis, symptoms, medications and more.

Working with our licensed therapists, you will create a plan of care to meet your personal health needs. Our home health care program includes various therapy services, such as:
- Physical Therapy
Physical therapists visit your home after falls, surgery, or injury to provide safety evaluations, exercise plans, and equipment or home modification advice. They also assist with worsening conditions like heart failure, Parkinson’s, or diabetes, helping improve strength, balance, mobility, and prevent falls. - Occupational Therapy
Our occupational therapists support recovery from surgery, worsening conditions like arthritis or Parkinson’s, stroke weakness, and pain. They assist with daily living activities, fine motor skills, adaptive equipment, energy conservation, and work simplification. In-home therapists help patients with physical or emotional challenges improve basic tasks like eating, bathing, and dressing. - Speech Therapy
ThedaCare’s certified speech-language pathologists help patients improve communication, swallowing, and language skills. Home Health Patient Telemonitoring
Stay connected and in control of your health with ThedaCare’s Home Health Patient Telemonitoring program. This free remote monitoring program includes:
- Easy-to-use biometric devices (scale, blood pressure monitor, pulse oximeter)
- Customized tablets featuring medication reminders, educational videos, and daily symptom surveys.
A registered nurse reviews your health data daily, provides timely alerts for any concerns, and coordinates with your care team as needed. You also have 24/7 access to nurse support. This remote monitoring helps improve outcomes and reduce hospital readmissions.
Patients can be referred to remote patient monitoring as a standalone service.
Home Infusion Services
