On Jan. 1, 2026, Holy Family Home Health Care will integrate with ThedaCare At Home. The integration is part of Froedtert ThedaCare coming together as one health system. The vision of this transition is to enhance access to care and provide additional services such as hospice and palliative care for more people and patients in surrounding areas, including Manitowoc, Sheboygan and Kewaunee counties.
“When a person is recovering from surgery or managing an illness, they often prefer to access health care services and products from the comfort and convenience of their own home,” said Kori Krueger, MD, Senior Vice President and President of the North Region Physician Group. “This personalized care in a comfortable and familiar setting can be a vital part of the healing process.
Home health care can be an alternative to an extended hospital or nursing home stay and can include care for most ages. Some need temporary or ongoing assistance for chronic conditions, such as congestive heart failure, chronic obstructive pulmonary disease (COPD), hypertension, cancer or diabetes.
Increasing Need for Home Health Services
A large portion of patients includes older adults. Adults age 65+ account for one of the most rapidly growing segments of the population. By 2030, it’s expected that one in five Americans will be 65 or older, with projections indicating that the population aged 65 and above could reach 95 million by 2060.
“The demand for home care services is rapidly growing due to an increase in the aging population, changing patient preferences and technological advancements,” said Brian Graf, Vice President of Froedtert Holy Family Memorial Hospital. “We are proud that our health care system is addressing the existing need in the communities we already serve, and expanding access in the region as well.”
By integrating Holy Family Home Health Care with ThedaCare At Home, Froedtert ThedaCare aims to coordinate a seamless transition for patients and families. Current patients will be notified and receive appropriate details through patient communication.
The integration is expected to be finalized and effective on Jan. 1, 2026.
