Clinic embraces new brand with local health system sponsorship
On July 1, UW Health Fox Valley Family Medicine will officially become Mosaic Family Health. The clinic, which serves more than 6,000 patients at its downtown Appleton location, is home to the Fox Valley Family Medicine Residency Program. Mosaic Family Health is sponsored by Ministry Health Care and ThedaCare.
Additionally, the residency program is transitioning its academic affiliation from the UW School of Medicine and Public Health to the Medical College of Wisconsin. The change is effective July 1. The program’s name is not changing.
“Our new clinic name and look reflect who we are as a community-based health care center and as the clinical home of a significant physician training program,” said Lee Vogel, MD, executive director of Mosaic Family Health. “The fresh brand is giving us renewed energy to continue providing compassionate, holistic care for individuals and families. We’re excited about our future!”
Although the longstanding clinic has a new name, patients can be assured the clinic’s accessibility, convenient same-day appointments and high quality of care are not changing. With its unique care model that allows patients to be seen by a doctor studying to become a specialist in family medicine, as well as by a faculty doctor, the clinic provides longer appointments, quality care and personal attention. Deep respect for patients’ participation in training doctors characterizes the clinic’s approach to care.
“By partnering with MCW and our community health systems, Mosaic Family Health and the residency program are addressing Wisconsin’s pending physician shortage,” said Allen Last, MD, director of the Fox Valley Family Medicine Residency Program. “Each year, about 50 percent of our graduates remain in northeastern Wisconsin. We’re proud and grateful to be part of this community.”

Mosaic Family Health is home to the Fox Valley Family Medicine Residency Program, a community-based affiliate of the Medical College of Wisconsin. The clinic is supported by its two local health systems, Ministry Health Care and ThedaCare.
