Medications to Induce Sleep – Dosing Reduction or Therapeutic Interchange

To reduce the risk of oversedation and patient falls, the P&T committees and Order Set Governance Committee have approved changes to medications used to induce sleep. Our preferred medications are melatonin, trazodone, or zolpidem. This strategy is approved to reduce the risk of oversedation and patient falls. This will also reduce therapeutic duplication when more than one medication for sleep is ordered. We have a new panel of single select medications to use in order sets. 

The order panel will present as an alternative alert when individual specific medications are ordered.  Providers may circumvent this substitution should a specific need arise (eg, circumstance where a patient is maintained on a hypnotic agent and is not served with the substitution.) The autosub document on Sharewell has been updated to reflect changes in medications to induce sleep. Consider this updated therapeutic interchange when entering orders for medications to induce sleep.
