System Root Cause Analysis (RCA) Process Being Refreshed

Historically, Root Cause Analysis, or RCA, has had a negative connotation attached due to phrases that accompany it such as “Investigation”, “Interview” or “Peer Review”. These words tend to feel criminal in nature and that individual fault seeking is the goal.

This is not the case. The goal of analysis is always to look for system process opportunities that will result in safer care for the next patient or a safer environment for our team members and providers to practice in.  

As we begin to roll out an updated process this fall you will start to hear different nomenclature. We will stop using the term” investigation” and start using “case review”- for truly, we review cases to look for what system processes may be deficient, non-existent or just plain not working any longer. This is how we become highly reliable: by creating processes and workflows that are repeatable, reliable, and deliver a consistent outcome- in this case Safe Care.
