These words tend to feel criminal in nature and that individual fault seeking is the goal. <br><br>This is not the case. The goal of analysis is always to look for system process opportunities that will result in safer care for the next patient or a safer environment for our team members and providers to practice in. \u00a0<br><br>As we begin to roll out an updated process this fall you will start to hear different nomenclature. We will stop using the term\u201d investigation\u201d and start using \u201ccase review\u201d- for truly, we review cases to look for what system processes may be deficient, non-existent or just plain not working any longer.
